Yang 2024 SR/MA (8 RCTs n=357 metabolic disorders) — TG and total cholesterol modestly reduced; LDL non-significant. Korotkov 2024 (Antioxidants 13:145) — 3-month electrolyzed HRW RCT in MetS/pre-MetS improved glucose; HbA1c; cholesterol; and redox markers. HYDRAPPET 2025 (PMC12300559) — 8-week HRW in obese adults reduced cravings; improved sleep; lowered LDL; raised plasma GLP-1. Elite-athlete crossover RCTs (Botek 2024 fin swimmers; 2024 handball/skeleton trial) confirm muscle recovery and reduced soreness; H2 is NOT on WADA 2024 prohibited list. Respiratory: Frontiers Immunol 2024 SR/MA — anti-inflammatory effect of high-concentration H2 inhalation in lung disease (asthma; COPD; ALI/ARDS). Cancer adjuvant: 2023 PMC10152878 SR — quality-of-life and chemo-tolerance signal but no survival data; 2024 Springer J Cancer Res Clin Oncol review reinforces adjuvant framing. Delivery matters: inhalation delivers ~4000× the H2 of one 500 mL water bottle and is the route most respiratory/cancer-adjunct trials use; oral water is cheapest but lowest-dose. Bottom line: real basic-science mechanism (Ohsawa 2007 Nat Med); modestly positive but heterogeneous human evidence; persistent COI concerns; better than placebo on several metabolic/recovery endpoints; still not a foundational longevity intervention.
Mechanism
H2 selectively neutralizes hydroxyl radicals and peroxynitrite (the most damaging ROS) without affecting signaling ROS like H2O2 and NO; small molecule with high tissue/membrane/CSF penetration; modulates NLRP3 inflammasome and Nrf2 antioxidant pathway
Dose & route
Hydrogen-rich water 0.5-1.6 ppm at 1-2 L/day; OR H2 tablets (modern Mg-based tablets transiently reach 5-12 ppm); OR inhalation 1-4% H2 gas 1-2 hr/day (highest absorbed dose by ~3 orders of magnitude vs. water but cost-prohibitive)
